Insurance Information

Our office participates with most major insurance providers in the region. If we participate with your plan, we will file the claim for you and wait for payment. However, co-payment is due at the time of your visit. Proof of insurance and photo ID are required at the first visit. If we do not participate in your plan, we will provide you with a claim form to submit to your provider for reimbursement. Uninsured patients are welcome, yet will need to pay a deposit prior to their visit and full payment will be required on the day services are rendered. Payment can be made by cash, check or credit card.

Insurance guidelines can be confusing, often referrals are required before visits to labs, physical therapists and similar. We encourage you to make yourself familiar with your plan’s regulations, for management of medical reimbursement is ultimately your responsibility.
